Senior Services of Southeastern Virginia
- To access a service, please contact SSSEVA at (757) 461-9481 and ask to speak with a Resource Specialist. The person or caregiver seeking services must be on the line if a professional is calling with a referral, as the Resource Specialist must have specific information and consent from the individual that they wish to receive the service. Services may change without notice based on availability of funding.
- Medicare Benefits Counseling – assists Medicare beneficiaries with education and enrollment counseling for Medicare Part D, Medicaid, long-term care insurances, private supplemental health insurance, and public benefits such as SNAP (Supplemental Nutrition Assistance Program) and LIHEAP (Low income Home Energy Assistance Program).
- Options Counseling – Senior Services’ Option Counselors help seniors live independently by performing a phone or home-based assessment and then working with individuals to develop a person-centered plan of care. Options Counseling includes explaining resource options for physical, medical, financial, or emotional needs; coordinating with other agencies as necessary and providing resources and support to existing caregivers or linking them to the Caregiver Specialist.
- Caregiver Specialist – Provides care coordination to seniors who have caregivers, working with them to obtain the services and supports needed to allow the senior to remain in their residence of choice and to help alleviate caregiver burden and burnout.
- Care Transition Coaches – Certified Care Transition Coaches work with clients who are returning home after a hospital stay to gain the knowledge and tools necessary to prevent avoidable readmissions. They help the clients understand their medicines, identify symptoms and red flags, and provide tools to help clients communicate with their doctors and other healthcare professionals.
- I-Ride Transit – This service provides medical transportation by appointment, and rides to senior center wellness and nutrition sites. I-Ride Transit can also serve special transportation needs. In addition, I-Ride Transit offers fixed-route transportation in Smithfield, Franklin, and the Wards Corner area of Norfolk.
- Senior Center Congregate Meals – Good nutrition is important for active aging, but some seniors do not have easy access to freshly prepared and nutritious meals. Senior Services offers a variety of delicious meals along with recreational activities at our senior wellness centers located throughout South Hampton Roads and Western Tidewater.
- Meals on Wheels – This program is intended to provide nutritious meals to homebound seniors who cannot prepare meals on their own and have no one in the home able to prepare them. Senior Services can arrange for home delivery of meals that comply with the Dietary Guidelines for Americans and are approved by a registered dietitian.
- Wellness Programs – One key to successful aging is the ability to stay informed and active. Senior Services wellness programs include: A Matter of Balance, Chronic Disease Self-Management, Diabetes Self-Management, and more.
- Senior Companion Program – Senior Services administers a Senior Companion Program (SCP) that partners senior volunteers with adults living independently who are in need of companionship and a helping hand. Options Counselors will assess the client’s condition, help develop a care plan, and work with the Senior Companion Program Manager to locate an appropriate companion.
- Senior Advocate Ombudsman – The ombudsman is trained to investigate complaints, mediate issues, and provide counseling and community education about nursing homes, assisted living facilities, and community-based care services.
- Personal Care Program – After an illness or during a period of convalescence, Senior Services’ short-term Personal Care Program can help with bathing, food preparation, feeding, and other self-care tasks. Options Counselors will assess the client’s condition, help develop a care plan and arrange for the services through a licensed home health agency.
- Homemaker Program – During periods of illness or recovery, Senior Services connects individuals to a licensed professional who can help with such tasks as light housekeeping or preparing a meal. Options Counselors will assess the client’s condition, help develop a care plan and arrange for short-term services through a licensed home health agency.
- Senior Cool Program – This program, which runs June 1 through the last working day of September, provides fans or air conditioners to eligible seniors who need help cooling their homes. To qualify for this program, sponsored by Dominion Energy, seniors must be 60 or older, in need of additional cooling at home, with income at or below 150 percent of the current year’s federal poverty level. The fans and air conditioners are issued on a first come basis.
- Veterans Services – In addition to any of the above services for which a veteran who is 60 or older qualifies, in partnership with Bay Aging, is an Options Counseling provider for veterans who participate in the Veteran Directed Care program offered through Virginia Veterans Administration Medical Centers.
Department of Human Services
Adult Services are designed to allow an adult to remain in the least restrictive
setting and function as independently as possible by providing supportive
services. Adult Services are provided to adults age 18 and older who have a
disability and to their families as appropriate
Norfolk Department of Human Services – Adult Services are:
Long-Term Services and Support (LTSS)
Nursing Home Pre-Screenings: Individuals who are Medicaid eligible or will be Medicaid eligible within 180 days of placement and who are seeking Medicaid long-term care services.
Personal Care: Services that may include assistance with bathing, dressing, toileting,
meal preparation, light housekeeping, laundry, medication reminders, and supervision. Personal care services are Agency Directed or Consumer Directed.

Assisted Living Facility (ALF) Assessments
Assessments are completed for persons who needs to move into an assisted living
facility and will be eligible for Auxiliary Grant payment to help with the cost of care in the facility.
Companion Services
Companion Services are available to income eligible and disabled adults who need help with basic daily living activities such as light housekeeping, meal preparation, laundry, bathing and dressing.
Adult Foster Care – A community assisted living option
Adult foster care is an optional program that provides room, board and supervision and special services to an adult who has a physical or mental health need. Overall, the idea is to provide a home-like environment with social stimulation and daily care so that adults with a disability and seniors can maintain autonomy.
Adult Protective Services (APS)
APS offers protective services to safeguard and defend adults from abuse, neglect, or exploitation.
